A U.K. coverage decision showed how eligibility rules can narrow access
Lilly wanted cover from a BMI of 30. The watchdog set the bar at 35 with a weight-related condition, and called the wider group poor value for the NHS.
In 2024, England’s health-technology body issued draft guidance supporting tirzepatide for a narrower subgroup of people with obesity than the manufacturer had proposed. The decision was tied to the National Health Service and its approach to cost and evidence.
The example shows that an approved medicine and a publicly funded coverage recommendation are different decisions. Eligibility rules can set boundaries around who is included even when a treatment has been cleared for use.
This was U.K. draft guidance, not U.S. policy or an individual treatment rule. Its thresholds and assumptions cannot be carried over automatically to another health system.
